Perhaps it is too late in the day, but I am confused.

I have a pt who had bacterial endocarditis in the past (Nov 2013). Is here for a f/u (has been seen every 3 months since Nov). Also being followed for hypertension and a PDA repair.

The physician is choosing the bacterial endocarditis as dx, but it is resolved and has been for months. He is still following the patient and performing an echo to evaluate the heart for recurrent infection, but the note clearly states, "no current infection".

I understand the patient is being followed for this, but since it is not present, how does it get coded?
