Looking to see if anything is billable here...

Patient came in due to retained nuclear fragments after cataract surgery. However, they were unable to remove any fragments [via multiple attempts/methods] and this at one point resulted in immediate iris prolapse in this patient with floppy iris syndrome [which they corrected].
