I keep getting a claim denied on a Medicare patient and the billing edits keep saying that " too many rev 521 charges on claim"? I'm not sure what I am doing wrong. The patient came for a regular sick visit and received a nebulizer treatment. I code 99214 and 94640. Please help!

Are you a rural health clinic?
From the presentation, The Basics of Rural Health Billing:
 Procedures performed on the same day as an RHC encounter will be bundled and ONE RATE will be paid for the entire encounter.

I am coding for a RHC. I understand what you mean by bundling but I can only use the E/M code and not the code for the neb treatment?
 
You can only have one 521 revenue code in any medicare part a submission. The fun part is trying to find the revenue codes for the other choices...labs revenue codes are 300 and injections are 771.
