When a patient presents with a list of either home BP or blood sugars, my providers think it's considered as part of the MDM data. from what I have read that seems to be summary of information from someone other than the patient. Would this go under ROS or HPI. I am confused.
 
HPI or ROS

It would depend on the note, but it is possible it could be used as either-but you can only count it once regardless of which area it is used in.
