Claim was billed with modifier XU on 19380 & insurance is requesting payment back stating modifier XU was invalid because recon & implant replacement were performed at the same session and same anatomic site resulting in edit. NCCI edits state modifier may be used so which modifier applies & what is above and beyond to warrant the modifier?
