CPT and the Medicare LCD is very clear on how you should code this scenario for an MBB but how do you code it for an RFA?

T12-L1- Per CPT and the LCD use code 64633

L1-L2 (during the same session) use code ______?

All the guidelines state are to use 64634 in conjunction with 64633 for cervical/thoracic and 64636 in conjunction with 64635 for lumbar/sacral. Reading between the lines it seems the most appropriate way to code the L1-L2 level is with 64635 (again, using the guidelines. It would be fantastic if the guidance was consistent with MBB's and we could use 64636)..

How do you/would you code it? We have tried 3 different scenarios and all get denied. I am just trying to choose 1 scenario to stick with, in my opinion that would be 64635 because we have the guidance in the CPT book and LCD to justify our coding.. Opinions?
