Wiki ride along for transport-medically necessary

If an NP must ride along on an ambulance transport, based on the medical necessity of the patient, can this service be billed? How should it be reported?
 
Transport

Unfortunately the only specific codes for medical transport are for pediatric patients under age 24months.

Is this continued critical care? You could use 99291-99292 code if you have at least 30 minutes.

Otherwise I would use the 99499 Unlisted E/M and have great documentation to send in support.
