Hi,

What will be Risk for MDM

assessment : Myopia
Plan : Prescription of glasses

I think it should be low but not sure need opinion.

If patient is in for rt exam and myopia is the only dx, you would report an eye code instead of E/M (either 92002/92012 or 92004/92014). MDM is not a component to use those office visit codes.
