When you are deciding on the Level of Risk (Box C) How do you determine which column is the best to use? Do you always choose the one that will get you the highest e/m level? If the patient has 3 stable problems and 2 of them are chronic, that would automatically get you to a moderate decision making level. Any advice would be greatly appreciated.

You are allowed to pick the most beneficial, also think about it moderate makes sense because if the patient has 2 stable chronic conditions most likely they are on RX meds which would also give you moderate in the last column. So if we are talking about office visits (moderate mdm) falls under 99214 you still need two out of three to appropriately bill at that level. Hope this helps Hope that helps
