We have received a denial for 92250 combined w/ 92014 and 92133, glaucoma patient. The Premera "what if" tool accepts the 99250 if I add a Modifier 59. I don't want to set up problems, any ideas? The "what if" tool also accepts 65222 and 92012 if I add a modifier 25, but again, not sure if it is quite appropriate. I would appreciate feedback!
