Pre op dx: Ventriculoperitoenal shunt malfunction
post op dx: Same
procedure performed:
1. Rivision of a proximal right ventriculoperitoneal shunt cathether
2. Use of neuronavigation
3. Removal of left frontal ventriculostomy.

I have cosen the codes; 62230, 61781 and 62220 what do you advice please.

Did you have a general surgeon help on this? I would at first say yes this is right - with hesitation. Would like to see the op report.
 
Without seeing the OR report, I would agree with the 62230 but have you looked at code 62160 instead of 61781? Also, from what you posted, he revised the catheter but didn't create a new shunt. I wouldn't bill 62220.
