COULD ANYONE TELL ME WHEN AN ROS IS MANDATORY IN THE RECORD? DO YOU NEED IT ON PE OR SICK VISITS? OR PERHAPS BOTH? DEPENDING ON WHAT?? THANKS:confused:
 
It's not mandatory. If the patient is in to see the doctor for a specific reason (problem focused), then there may not be a ROS. Just a FYI, though - if they do vitals during ROS, that counts as consititutional.
 
Maric-ann, Vitals would not count as anything within the ROS; that would fall under constitutional in the examination, because vitals are an objective assessment, not subjective.

For follow ups, If you are counting the history and not just using MDM and Exam to meet the criteria

99233 ROS 2-9

Actually, codes that require a comprehensive history require a review of all 14 systems, with at least 10 systems documented.
