does anyone have guidelines to state if it is acceptable to use the ROS to code ICD-9 even if the doctor has not written the dx in the assessment. would appreciate any help.

The ROS is just that a REVIEW of patient's systems ... it is not a diagnosis and no ICD-9 code should be assigned to the ROS alone.
