Hi,
I hope we are consider this as severity for HPI.

Severity may also be assessed by non-verbal signs of discomfort such as wincing, pacing, facial grimacing, moaning or groaning, writhing or constant shifting in
bed, restlessness, and agitation.
Severity is noted to be an adjective qualifier describing the degree of severity or intensity, i.e. worst, improving, compared to, unable to, mild, moderate, severe, or the patient feels well.
