Wiki routine annual exam vs abnormal pap follow up

Patient came to the office for annual exam in 2010, results lgsil, colp'd, and did not return to the office for 6 month f/u. Came back to office in 7-2012 and physician billed 992xx with abnormal pap dx. Insurance applied to ded and the patient says that this was supposed to be her annual pap exam. Is the doctor wrong here? Patient went on to say after the fact that she followed up with another dr and pap was normal however none of that is in the note in fact it mentions that no pap smears have been done since the abnormal.

If anyone can give me some advice, I would appreciate it.
