We have been providing RPM and CCM for patients and no matter what we do we keep getting denials for at least one of the claims stating they are incidental to each other even though they are separate programs with separate requirements and time. 99491, 99437 and 99453, 99454, 99457, 99458. Each of the review codes are calendar month based and we have tried submitting them on the same claim and separate claims and we get the same result even though they are all allowable codes. Any suggestions from anyone else here?
