A patient with improved back pain comes in to see the provider for a return to work exam. The provider notes the patient's back pain is improved, but not 100% and releases the patient to return to work. I have a presentation from an occupational health session that I found online that says to report the primary dx as the condition the patient has followed by code V68.0 as secondary. Which code would I report for a RTW exam? V68.01 for a disability exam or V68.09 for other issue of medical certificates? Thanks.

Indication being "RTW exam", I'll code to V68.01as PDX + SDX 724.5

PDX-Fitness for Duty/ Evaluation of worker to assess fitness to return to work
SDX-for pertinent medical problems

I'd NOT code v68.09 -as it is assigned if the primary reason for the encounter is solely to obtain a medical certificate, and the provider does not evaluate or treat any symptoms, conditions, or diseases.
