how would you report additional rubber band ligation done in office during the 10 day global?
 
If it was discussed with the patient prior to the bandings being done that it is possible/probable that they will be done in sequence (i.e. not all of them at the same time) i would use a 58 modfier on the subsequent bandings. Most people have at least 3 prominent hemorrhoids and most surgeons do not band them at the same time as it would be to painful (i have seen some surgeons do it and the patient's are miserable afterwards) . An argument can also be made for modifier 79 as it is a different hemorrhoid in a different part of the rectum (I assume) that is being banded. I would lean toward 58 as long as the criteria were met.
