Hi and Happy Thanksgiving Everyone!

QUESTION?
Can we bill 99211 for RX.REFILL if there is no notes in the patients chart OR it is necessary to write notes in the chart in order for us to bill for 99211? BUT we have a copy of the RX. that is given it is substantial?
