I have a general question about the excludes 1 note under S00; does the excludes 1 note mean I cannot code S00.92XA & S01.511A on the same patient?
 
Yes that is what excludes 1 means. However there is an exception in the guidelines that allows you to use both codes when documentation can support that that these do exist as two independent diagnoses, in other words, if you can support two different wounds of different areas. So it may need to be appealed if the payer edit rejects it for mutually exclusive diagnosis.

S00.92XA- Blister (nonthermal) of unspecified part of head, initial encounter
S01.511A-Laceration without foreign body of lip, initial encounter
if you find documentation supporting both the conditions individually then you can bill those even if excludes 1 note is give as per new updates.
