I am getting a lot of confusing information on this code and how to use it - the CPT says to bill it in addition to the primary procedure. Some payers will pay it, some will not. Has any one had experience with this code?

How do you calculate the fee to charge? I have heard charging a flat set amount or charging a percentage of the procedure fee.
