Does S4016 include IVF, ICSI, Cryopreservation, Cryostorage, and PGT Biopsy?
 
It would depend on the payer. Which payer are you asking for? For specialty payers like Progyny the S4016 does not include ICSI (89280/89281) or PGT biopsy (89290/89291) but they normally authorize these additional services when the approve a S4016. For payers like Aetna and Optum/UHC, the S4016 is used for an FET and S4011 is used for IVF. They also do not include ICSI, cryo, storage, biopsy. Separate auths would need to be obtained for these.
