I am researching this code to see if there is auditing available. The code is a case rate meaning t's used to count the number of IUI cycles, regardless of the individual services within each cycle. Does this mean that the following ind codes would bundle? I mean should they not bill one of the following if they have billed S4035? I just don't have a good grasp on the meaning of the case rate.
  • 58321: Artificial insemination, intracervical
  • 58323: Sperm washing for artificial insemination
  • S4035: Intrauterine insemination, stimulated
  • 89250: Culture of oocyte(s)/embryo(s), less than 4 days
  • 89251: Culture of oocyte(s)/embryo(s), 4-7 days
