Is it correct to bill 58720 and 49320- 59? Laparoscopy attempted- but proceeded with open laparotomy . Is laparosopy "distinct or independent" from salpingo- oopprectomy?
 
noyou cannot bill for both the scope and the open procedure, you can bill the open procedure only. however if the documentations supports that there was great difficulty for some reason and significant time and skill was involved as a result of needing to convert to the open then you can append a 22 modifier to the open procedure code.
