Is it possible at all to split the RHC/Coronary Angiography code (93456) if the two procedures were performed during different encounters (with separate reports) by the same physician on the same day? I was thinking I could use the -58 modifier for a staged or related procedure, but Code Correct tells me I cannot use any modifier with the code pair of 93451 and 93454.

For an unplanned returned to the cath lab on the same day, I would place a 59 modifier on the 2nd procedure. Do make sure medical necessity is documented as you will most likely have to appeal.
