In 21 years, I have never experienced this before and am looking for help. I can not seem to find my answer clearly in the CMS claim guidelines manual Chapter 12.
My provider rendered a surgical excision (11606) and intermediate repair (12034) 0-10 global , and unusually due to time, had to have our PA render the remaining ED&C (17266) 0-10 global, that still need to be performed.
My thought it that we would just submit two different claims for each individual provider, since our PA does bill to Medicare. However, I am unsure and didn't know if any modifiers needed to be appended?
