Does anyone know if the 3 additional services has to be done in addition to the BP, Weight and Fetal Tones or a least one has to be documented?

Per SC Medicaid policy:
Antepartum Visits (99213) with Additional Services - Antepartum care includes cntinuing physical exams and recording of weight, blood pressure, and fetal tones. The additional services necessary for use of this enhanced antepartum code includes:
* Follow-up on referrals
* Follow-up on missed appointments
* Continued health education

My first thought is at least one of the above elements has to be documented to qualify for the level because not all patients are going to need follow-up on missed appointments. I know they will need WIC referrals, etc and continued health education at some point.

Please help!!!
