Does anyone know how to code the revision of a laparotomy scar? I know the incision is included in all surgery procedures but is there a separate code for revision? :p

Depends

It depends on the operative note and the procedure(s) described therein.

Usually scar revisions are coded in one of the following ways:
1) excision of benign lesion (plus layered closure, if performed)
2) complex wound repair (IF extensive undermining is described)
3) tissue rearrangement (i.e. skin flap; this includes the excision of the "lesion").
