Wiki Screening and Diagnostic mammos done on the same day

I know that a screening and a diagnostic mammo can be performed and billed on the same day, my questions is about documentation. Does there need to be two seperate reports for each study? If the radiologist states "There is a focal symmetry in the medial aspect of the left breast which is only visualized on the cc view and did not persist on the spot compression view" and this is the only documentation reported for the diagnostic unilateral (and let me add it is contained in the report for the screening mammo) is this enough to support billing both the screening and the diagnostic? Please give feed back as this is an on going challenge in my office.
