When you do a colonoscopy with the pre operative diagnosis of screening colonoscopy and while doing the colonoscopy you find that the patient has a problem such as diverticulosis, do you assign the diagnosis for the screening colonoscopy in addition to the diagnosis for diverticulosis, or would you only use the diagnosis for diverticulosis?
 
Per the Gastroenterology report you would dx the Screening primary (V76.51)to indicate that the patient presented for a screening and then the findings/pathology secondary (562.10). The CPT should reflect the procedure that was done, bx, polypectomy, ect.
