I'm thinking I might have to go unlisted code on this, but I want to appeal to the hive mind in case I'm missing something.
Long story short, while high on drugs, patient removed both of his testicles. My doctor saw him when his scrotum was "traumatically open and full of clot" - he removed the clot (I'm thinking 54700 for that) and then the doctor identified and tied off both cords with 0 silk. He then packed the wound. He did not close the wound, which makes me shy away from a wound repair code, but I am not abject to it entirely.
