I'm looking at CPT 64999 for this procedure. As the provider is not doing anything with the generator or leads themselves all he is doing is deepening the pocket and moving the generator. The office is fighting back with CPT 63688. "The battery upper portion of it is causing some discomfort for him. I discussed with him to place the battery deep per I explained to him the risks and benefits including but not limited to bleeding infection nerve damage. I explained to him that if infection happen I will have to remove the whole entire system patient agreed with the plan he wants this to be repositioned again. I will ask for insurance approval for spinal cord stimulator battery reposition."

Any input would be helpful.

Hi there, your scenario seems to fit an example from a June 2015 CPT Knowledge Base article which describes the appropriate use of 63688:

Another example is in the event the generator is resting on the pelvic rim and is moved due to pain. In these two specific circumstances, the work is considered revision of an existing generator so only code 63688, Revision or removal of implanted spinal neurostimulator pulse generator or receiver, should be reported.
