Wiki Secondary diagnoses

I am always confused about secondary diagnosis coding. I have the CCs rule related to coding secondary diagnoses when they are listed as a symptom followed by comparative/contrasting diagnoses. However, what about a secondary diagnosis listed as "chest pain, rule out angina" or "dysphagia and slurred speech, rule out CVA"??

If you are billing physician or outpatient you cannot code rule out dx you code only the symptoms.
