I code cardiology diagnostics. In this example I need a covered DX for a limited echo. There are no indications other than "watchman f/u"
Can I use the same DX from the watchman procedure report which was Afib if its not mentioned on the echo report or order?
 
Look into Z45.09 or Z01.89.
You shouldn't go back to get a dx from another encounter unless for ancillary facility has a policy in what circumstances you may.
