Can a patient choose to be changed to self pay for physical therapy charges to preserve Medicare dollars? Do they have to sign an ABN?

If the patient doesn't want insurance billed we have them sign an agreement form. It's called Request for Restrictions on Use/Disclosure of PHI to A Health Plan For Payment and Health Care Operations. We do not bill the insurance so an ABN is not required. We have them pay prior to the procedure.
