Does anyone have a good, clear, concise definition or explanation for Separate and Identifiable?
 
If you are talking in relation to the 25 modifier?
Every procedure or service has an assessment/eval and resources that are necessary in order to adequately perform the service. To be separately identifiable there must be an assessment/eval or resources used that go over above and beyond the need to perform the service and the procedure or service must be identified in a stand alone note or paragraph, not as a part of the assessment.
