Hello - provider performed 31237. He also has this documented:

PROCEDURE:
After informed consent was confirmed, the patient was taken the procedure room. The septal perforation was confirmed on endoscopy. The perforation was far too small for conventional septal buttons that are commercially available. As a result a custom implant needed to be created. Silastic sheeting was used and it was trimmed to an approximately 1 cm diameter circle. 3-0 Ethilon suture was used in a mattress fashion to form a septal button. Once this had been completed the button was placed under endoscopic guidance. The patient tolerated the procedure well. There were no complications.

Is this billable or included in 31237 or perhaps 30999?
