New patient enters the clinic with R medial aspect of knee with approximately 12 cm interrupted linear wound/ scrape. The middle aspect / segment is slightly open but not currently weeping. There is about a quarter size erythema around this area, minimally tender, non fluctuant, but there is a quarter size mass underneath not entirely well circumsized. Possible edema from injury vs abscess.

Note: localized infection noted; discussed treatment of I&D vs watch and monitor and warm packs till friday (2 days from today), as patient believes it is getting better.
Advised bacitracin Bid and warm packs over site.
RTC if mass under mid portion not diminished in size or if worsening by Friday and will I&D.
Patient verbalizes understanding and agrees to plan of care.

Looking for a good explaination. Would this beT81.30xS (sequealae) or T81.30xA ( initial encounter)?

I would say that sequelae would not be appropriate, as that is used when the acute phase is over and there are after-effects. It doesn't look like the acute phase is over.
