A new radiologist we have is using this phrase quite a bit, sometimes it is the only thing in the "impression"and I am not sure what to do with it: Mild sequela of small vessel ischemic disease.



Any thoughts?
 
Could be a documentation error? When in doubt, I always send back to reading/interpreting provider and as to clarify. Our system will provide multiple possible "indications" when they begin to document, much like how google has the "suggestion" search while you type in the search bar.
