I have a general question about the sequence of icd-9 codes. If you have a patient that is coming in for a procedure ( we are an ASC) and they have several symptoms listed in the indications and also have some findings how would you determine the correct order to list the icd-9 codes on your claim form. This is just a general question that has been asked of me that I am looking for opionions/suggestions. Thank you.
 
If this were me i would code the findings correlating to the procedure that is being perfomed in your facility primary.

Patients may have many diagnoses but the corresponsing diagnosis (reason) they are having a procedure in your facility should be primary.
