When a pt comes in for sexual assault in adult and final dx is sexual assault of adult. would you take this as a confirmed or suspected?
 
Confirmation comes from the provider. I would say that if there are any phrases like possible, consistent with, indications of, and so forth, it should be coded as suspected--one of those extremely rare times in the outpatient setting we can code for "possible" diagnosis. Inpatient, you might want to talk to someone higher up, because you might have to query the provider to commit one way or the other (that it's either confirmed or they're unsure.)
