I'm looking for suggestions on what is the required documentation for an ED physician when the visit is shared with a NPP. Medicare is rather vaque and simply states the physician must have a face-to-face encounter with the patient.

What are other ED groups requiring of the physicians? To you require documentation of a History, exam and MDM or is one of these E/M components sufficient?
