What CPT code is used for an SI Ablation?

Also, Where can I find the coding guidelines for documenting Comorbidities when the patient is seen by a specialist and the specialist codes the dx for what they are being seen for? EX: Pt seen for spine issues and the pt has the following dx 724.6,721.3 and 722.52 plus the pt has diabetes which is also coded: 250.00.
