I was recently denied, upon appeal, for an illegible signature. I want to implement a sign.log/attest stment, but would like to review what other practices have put into place. Our physician's are requied to initial all calculations, places, simulations, devices, etc. to show participation in the planning of radiation. From the CMS guidelines, initials are okay if over a type/printed name. This will not be the case. Any suggestions? What does the "statement" need to say? Can is be just a sentence stating these initials, for all intent and purposes, is sufficient for the signature/review of Dr. so and so.
