Can anyone direct me to something that states whether or not Medicare or any other insurance company REQUIRES a physician signature on a Laboratory Test Request or Order?
CLIA requires a written request but does not state that they must be signed for performance of testing. We are developing a Laboratory Billing and Compliance committee and this is the first issue our hospital billing department wants addressed.
