Is a provider's signature required on the encounter forms? Our providers circle the service charge and write in a diagnosis. Should they also be signing or at least initialing the form?
 
I know of no rule that states the provider must sign the superbill/encounter form, however they must sign the progress note. The coding must be done using the progress/surgical note for every claim created.
