When using the simple visit coding..does the coder need the physician order when auditing the codes the system has pulled in?

If the visit is errored out because there is no diagnosis. Does the coder need the signed order to code from?
 
The coder must always have the narrative office note in order to determine the correct codes for a claim, either for initial creation or auditing.
