Wiki Simple vs Intermediate suture?

We're having a discussion at work & I wanted impartial input. Apparently depending on what company people bought their CPT books from, we have definitions of Intermediate as "Single-layer closure of heavily contaminated wounds that have required extensive cleaning or removal of particulate matter" and also "can involve a single layer closure with extensive cleaning or removal or particulate matter" with no mention of contamination. As we see a fair amount of Procedure Notes indicating Excessive Cleaning but no contamination, this is making for inconsistancies.

What is it supposed to actually be? Please reference justification if possible.
