Wiki Single Page Sign Off

We are a small (13 bed) CAH hospital, and I have recently been approached about changing to a single page signature page for different admission types, i.e. Inpatient, Swingbed, Obs as well as outpatient clinic visits. The idea is to have a page that states "I received or was offered the following documents at the time of my admission to the facility and had them explained to me in a language I could understand" - followed by a list of documents such as Admission Agreement and Treatment Consent, Release of Information, Notice of Privacy Practices, Patient rights and Responsibilities, Notice of Visitation Rights, Physician Availability Disclosure Rule, etc.

I would appreciate any input as to whether this would be acceptable, or if anyone see any problems with this. Do I need to go as far as running this by the hospital attorney? Or is this my call to make as the HIM Director?
