Does anyone know if there is a standard for prior authorization of sinus debridement? How many visits do you have preauthorized?

It depends upon the payer. Some do not require an auth for minor procedures like this. If within global of a 90 global procedure done at the same time as FESS, add 58 modifier.

Your doctor would be the best to ask as to how many of these are needed post op. I have 2 sinus surgeon's that use different post op times.
